Obverse
The obverse shows a portrait of the Queen, seated facing on a throne, holding a sceptre and mace, in the style of the Henry VII gold sovereigns, with the inscription.

Reverse
The reverse shows a tudor rose within a represention of a crown, with the inscription:-
VIVAT REGINA
1952 1977
The inscription translates as (Long) Live The Queen
Edge
The edge bears a platinum hallmark, with the sponsor's mark (initials) JRC, and the serial number of the medallion. This hallmark includes the special "Silver Jubilee" hallmark which was in use for the jubilee year only.
The three examples of this medallion we have seen bore the serial numbers 7, 8, and 9.
